About this Event
We know that for a lot of patients and their families the transition from children to adult hospice services can be really stressful. They have often used the same services for years so they know and trust the staff. They can also be anxious about adult services being different and about all the other changes that happen when someone turns 18 years.
Getting transition right and understanding the needs of young people is vital. This face to face, joint training session run by Rainbows and LOROS aims to offer a better understanding of how we can support young people and their families to feel more confident and comfortable about accessing accessing adult palliative care services.
Learning outcomes
• Understand what is meant by transition, both locally and nationally
• Have an overview of the differences and similarities between children and adult hospice care
• Be aware of the impact of transition on young people, families/carers and staff
• Understand the needs of young people
• An introduction to legal aspects of care for young people
• Understand the multidisciplinary approach to transition
• Gain confidence to support young people more effectively in a hospice setting

For all clinical facing staff at LOROS, Rainbows and other external agencies working in palliative care
