Aims of the day

• To provide delegates with an overview of a variety of services across BSol ICS providing care and support to young people and their families experiencing transition between paediatric and adult services in health, social care and education.
• Consider some of the ‘headaches, highlights and horizons’ associated with this cohort of young people
• Create an opportunity for networking with colleagues
• Generate a space for reflection of own practices and experiences
