-Duly filled-out registration form.
-Photocopy of receipts from mall shops worth P500 (single or accumulated). Receipts from NCCC Supermarket, NCCC Department Store, HB1 and Hardwaremaxx are excluded.
-Registration Fee of P100.
-Photocopy of childβs birth-certificate.
