Eileen Zenker LCSW, CCM - Regional Manager of Social at Senior Bridge for Social Work Care Managers in the NY metropolitan region. Prior to joining Senior Bridge, I worked at NYU Langone and Beth Israel Medical Center Much of my focus has been on complex discharge planning .I am very well versed on many community resources and regulations and understand the importance of balancing quality care, patient satisfaction and transitional care planning.
In addition to my leadership roles, I am adjunct faculty at Touro College School of Social Work and Hunter College Silberman School of Social Work. I have also been a mentor for NYU Silver School of Social Work for the End Of Life Certificate Program and Smith College End of Life program. I have presented at many national conferences, CMSA, American Society of Aging and NASW, in addition to presenting at many local conferences. I have a certificate in bio ethics and participate in teaching a yearly ethics course for the residents in rehabilitation at Rusk, and also the second year medical students.
