1. Assumption of Risk

I, the undersigned, acknowledge and understand that attending the event and/or visiting On the "Y" Sacramento involves inherent risks, including but not limited to, the risk of personal injury, property damage, or loss. These risks may arise from factors such as crowd behavior, alcohol consumption, accidents, or other unforeseen incidents.

I voluntarily choose to assume all risks associated with my attendance.

2. Waiver of Liability

In consideration for being permitted to enter and participate in events at I, the undersigned, acknowledge and understand that attending the event and/or visiting On the "Y" Sacramento involves inherent risks, including but not limited to, the risk of personal injury, property damage, or loss. These risks may arise from factors such as crowd behavior, alcohol consumption, accidents, or other unforeseen incidents., I hereby agree to release, discharge, and hold harmless On the "Y" Sacramento, its employees, agents, affiliates, and all persons or entities involved with the operation of the venue from any and all liability for any personal injury, property damage, or loss, arising from my participation or attendance.

This release extends to any claims arising from the negligence or gross negligence of the nightclub, its affiliates, employees, or agents, to the fullest extent permitted by law.

3. Indemnification

I agree to indemnify and hold harmless On the "Y" Sacramento and its employees, affiliates, and agents from any claims, actions, damages, or liabilities arising from my own actions or omissions during my visit, including any injuries I may cause to others or to myself.

4. Medical Treatment

In the event of an emergency, I consent to receive medical treatment if necessary. I acknowledge that I am responsible for any costs associated with medical care.

5. Conduct and Behavior

I agree to abide by the rules and regulations set by the venue and understand that failure to do so may result in immediate removal from the premises without refund. I acknowledge that On the "Y" Sacramento reserves the right to refuse entry or service to anyone at its discretion.

6. Alcohol and Drug Use

I confirm that I am of legal drinking age in the jurisdiction in which On the "Y" Sacramento operates. I acknowledge that excessive alcohol consumption, illegal drug use, or disruptive behavior is strictly prohibited, and the venue reserves the right to remove any individual who is violating such policies.

7. Photography and Recording Consent

I acknowledge that while I am at the venue, photographs or videos may be taken by our media team for promotional purposes. I consent to the use of my image and likeness in any promotional or advertising material without compensation.

8. Acknowledgment of Understanding

I have read and fully understand the contents of this Liability Release and Waiver Form. By purchasing a ticket/attending, I am signing this form voluntarily, and I understand the risks involved in attending the venue. I agree to comply with all rules and regulations.
