Join Us for the Science of CHW Sustainability Conference!

A full-day interactive conference focused on organizations based in Northern Arizona to explore the science, practice, and policy needed to build and sustain Community Health Worker (CHW) programs in the Northern Arizona region.


Sessions will feature:
  • Evidence-based and practice-informed CHW models
  • Real-world CHW program implementation strategies
  • Workforce and infrastructure development
  • Financing and reimbursement pathways
  • Policy approaches that support CHWs as essential members of care teams and community-based systems
