INTRODUCTION
Failure Mode & Effects Analysis (FMEA) is a tool for identifying all potential modes of failure, possible causes and effects of failure on the system or interface. The purpose is the early identification of failures so that they can be eliminated or minimized through design correction at the earliest time. FMEA enables improvements and corrective actions to be implemented for design, process control, maintenance and service.
PFMEA (Process Failure Mode Effects Analysis) focuses on processes. PFMEA is a qualitative tool, a living document, that zooms into current processes to identify improvement points. PFMEA is part of the larger failure mode and effects analysis (FMEA) family. A PFMEA will break down an organization's maintenance process into its individual steps and methodically examine them for potential risks or failures.
Learning Objective
At the end of the course, participants will be able to:
- Describe the FMEA history, terms, standards and types
- State the benefits and advantages of using an FMEA
- Describe the composition and responsibilities of an FMEA team
- Explain when an FMEA is needed and the timing involved
- Describe the basic information and inputs required in each column of an FMEA form
METHODOLOGY
Group activities, exercises, videos, sharing of knowledge and case studies will be used to provide participants with learning opportunities.

Recommended for Development Coordinators, APQP Team Members, Process Engineers, Quality Professionals, PPAP Coordinators, Project Managers, and other manufacturing roles related to quality assurance.
