How can people of all ages be better prepared for a medical emergency or medical decisions that can affect the rest of one’s life?
About this Event

Plan for the joyful and best life, but prepare for the unexpected.

We will discuss what Advanced Directives are and why these legal documents and other ready information actually tell what a person loves and values in life, and if there is ever a serious health situation or a need for help, how these prepared documents can ease the emotional and practical challenges for yourself and the people who care about you,

We will talk about why having discussion of life and health challenges can be helpful to you, your friends and family in facing the good and the difficult times, as well as help open discussions with your children, partners, parents or friends. Advanced directives are important for every adult. We will discuss why everyone over 18 needs a Health Care Proxy, and older adults can benefit from having paperwork completed and accessible when they are not under big pressure in an urgent situation to consider and sign documents.

You will receive a guideline and envelope for a To Go Emergency Information packet that will hold important documents that will help you in a medical emergency.

We will have time for questions and will have the paperwork for the New York State Health Care Proxy, so that anyone who wants to can complete and have witnesses sign the Proxy, if they feel ready and want to get it done, at the end of the session.

Zoe Moffitt, RN-BC, is an RN, a Geriatric Care Manager and Board Certified Gerontological Nurse, an artist, a gardener, a Reiki master and a Hudson Valley trail walker, who works with individual elders, families and friends to help make the best of their later years, with whatever brings them joy, optimal health and comfort, even with chronic diseases or difficult transitions. Zoe has had experiences with clients, friends and family when Advanced Directives (or in a few cases, their absence) made a huge difference in a person’s care and recovery, and she has had these discussions with her two adult children and son in law, as well as with her own parents. Her practice is Integrative Elder Care RN, PLLC.
