Ensure your healthcare preferences, personal values, and life goals are known and honored. This workshop, hosted by the Fox Valley Advance Care Planning Partnership, provides a supportive environment to discuss these important decisions and complete a Power of Attorney for Healthcare (POA-HC) document if desired.
Led by Respecting Choices-certified facilitators, the session will cover:
- Selecting a trusted healthcare agent.
- Understanding the legal document known as the Power of Attorney for Healthcare.
- Considering your medical wishes and personal values.
- Deciding who will speak for you if you cannot speak for yourself.
