CLIENT AGREEMENT AND DISCLOSURE STATEMENT
Thank you for your interest in working with me as a client in person or at a distance using phone/FaceTime/Zoom, whichever is applicable.
All are welcome. We do not discriminate on the basis of race, color, religion/faith, creed, sexual orientation, gender identity, gender expression, national origin or ancestry, citizenship, military status, age, income, background or ability/disability. If for any reason we determine it is not a good fit between practitioner and client we will recommend another practitioner for you if possible.
I am providing you with the following information so you can make an informed choice about your decision to engage my services. Please read this information carefully and let me know if there is any part you do not understand.
SERVICES OFFERED/THEORETICAL APPROACH
I offer my services as a complementary and alternative healthcare practitioner. I am not licensed, certified, or registered by any state as a healthcare professional, nor am I subject to licensure, certification, or registration.
OUTCOME EXPECTATIONS/RISKS & BENEFITS
By engaging in my services, you understand that there are numerous benefits possible through [Sound Healing], such as increased relaxation, relief from anxiety, improved sleep, and diminished pain sensation. Please note that it’s impossible to guarantee any specific results and these effects may vary depending on each individual’s response patterns.
HEALTH AND SAFETY
My services are not a replacement for proper medical treatment and/or care. Please consult with a qualified and licensed physician or other medical care provider before attending a session.
I do not provide services for clients younger than 14 years of age. All clients between 14-18 years of age must have parent/guardian consent and that parent/guardian must be in the room with the client at all times during their session.

Severe unexplained internal pain
Hemorrhage, atherosclerosis/ arteriosclerosis

LIABILITY
I, the participant, hereby agree to assume all risks associated with any outcome resulting from my participation in Sound Healing with Nicole/ Lemurian Lotus programs or classes or private consultation. Side effects may include, but not limited to: emotional release, lightheadedness, mild fatigue, physical tingling, and change in body temperature.
I, the participant, also hereby voluntarily release from liability for any financial, physical, or other adverse results of any other individual or organization associated with the activities for which I agree herein to participate with Nicole/Lemurian Lotus, including any organization rented for workshops retreats, or any other reason. I understand and agree that I am releasing not only these organizations but also their officers, employees, members, and independent contractors.
I, the participant, understand and agree that this agreement will have the effect of releasing, discharging, waiving, and forever giving up my right to bring any and all lawsuits or other actions or demands against any of the above-named individuals, groups or organizations, that I may have or have had contact with, whether past, present or future, whether known or unknown, whether anticipated or unanticipated by myself, arising from my attendance at this retreat/ event. This agreement constitutes a complete release, discharge, and waiver of any and all lawsuits, actions, or demands against the individual or organization associated with Nicole and Lemurian Lotus, their officers, employees, agents, volunteers, or independent contractors.
I, the participant, understand and agree that this agreement applies to personal injury, property loss or damage, or death, which I may suffer, even if caused by acts or omissions on the part of Nicole/ Lemurian Lotus or others.
I, the participant, understand and agree that this agreement will be binding on myself, my spouse, children and any guardian ad litem for such children, heirs, my assigns, and I, the participant, understand and agree that by paying for a healing session, I agree to release, indemnify, and hold Nicole and the above-named parties, its officers, employees, agents, or independent contractors harmless from any and all liability or costs, including attorney’s fees, associated with or arising from my attendance in any sessions with Lemurian Lotus (Nicole).

It is my choice to receive services from Lemurian Lotus, Nicole, and I understand that the practitioner may be using gentle sound and vibration during these sessions around me. I understand that my practitioner does not diagnose illness, disease, or physical or mental disorders, nor do they prescribe medical treatments or pharmaceuticals. I acknowledge that these sessions are not a substitute for medical
examination or diagnosis and that it is recommended that I see a primary health care provider for those services. I understand that I alone am responsible for informing my primary healthcare provider that I am receiving these sessions and inquiring as to whether or not they may adversely affect my current health condition. As such I will not hold my practitioner liable for any adverse effects of such sessions on any health conditions.
ACKNOWLEDGEMENT AND CONSENT TO RECEIVE SERVICES
By signing this document you agree that I have disclosed to you sufficient information to enable you to decide to undergo or forego the services I offer. You have considered all of the above information and have obtained whatever information or professional advice you deem necessary to make an informed decision. By signing this document you understand I am offering my services solely as a complementary and alternative health care practitioner and our relationship is not to be construed as medical treatment, psychotherapy, psychological counseling, or any type of therapy, nor is it a substitute for
these services. In light of this, you accept full responsibility for any and all risks associated with receiving sessions. You understand it is your responsibility to maintain a relationship with a healthcare professional. Further, you understand your consent to the nature of our sessions is given voluntarily, without coercion, and may be withdrawn at any time.
Future withdrawal of such consent may affect your ability to continue attending such sessions. You represent that you are competent and able to understand the nature of the proposed sessions and agree to be personally responsible for the fees related thereto. Payment is expected prior to or at the time of service. You have discussed
with me the nature of the services to be provided and you understand that I am not a licensed, registered, or certified health care provider in any state. You agree and understand that this Agreement is intended to be a complete unconditional release of liability and assumption of risk to the greatest extent permitted by law.
By purchasing a ticket, and attending this event, you knowingly, voluntarily, and intelligently assume these risks and agree to irrevocably release, indemnify, hold harmless and defend Nicole and LemurianLotus, LLC and their agents, representatives, consultants, and employees from and against any and all claims of whatsoever kind or nature, and for any loss, damage, or injury, including but not limited to, financial, personal, emotional, psychological, medial, or otherwise which you may incur arising at any time out of or in connection with your sessions.
APPOINTMENTS AND CANCELLATION POLICY
Local In-Person & Virtual Sessions: *A 25% non-refundable booking deposit is retained by Lemurian Lotus for all sessions.*

Within 24 hours of session- This is considered a cancellation and 100% of the booking fee is forfeited. You can purchase a new session through our booking form.
To Cancel: Send an email to nicole@[email protected]. Please allow 5 business days for the refund (if applicable) to show in your account.
Within 24 hours of session- 100% of the booking fee is retained by Lemurian Lotus .
