On Your Feet is a fall prevention program for adults over 60 at risk of falling or who have fallen recently. With education and intervention, falls can be prevented, reducing injury and preserving independence. Learn how to stay safe, improve quality of life, and stay independent.
Qualifications
- Age 60 or older
- Experienced a previous fall or have fear of falling
- Be able to walk (assistive devices such as walkers and canes are acceptable)
- Live at home (either independently or with family) or in an independent or assisted living facility
- Have the cognitive ability to participate
Program Overview
Fall Risk Assessment
- Identify individual fall risks
- Review of medications that may increase fall risk (e.g., sedatives, blood pressure meds)
Mobility Planning
- Tips on how to stay independent
- Guidance on using assistive devices
- Exercises to improve balance and flexibility for all fitness levels
Home Safety Checklist
- Identify fall hazards Tips for creating a safer home environment
- Simple modifications to improve safety and mobility
