To complete an application, please have the following for each person in your household applying for coverage:
Dates of birth
Social Security numbers
Immigration documents (visa, green card, etc.) if applicable
Most recent tax documents (W-2, 1040, 1099)
Paystubs or income details (Profit & Loss statement if self-employed)
Information on current health insurance (if any)
