What is NH Care Connections?
Established by the NH Department of Health and Human Services and the NH Department of Military Affairs and Veterans Services (DMAVS). NH Care Connections is a closed-loop referral network that includes providers that offer services such as healthcare, housing, employment, food assistance, mental health, utilities, education, veteran services, and more. NH Care Connections is being rolled out to all health care and human services providers and community organizations, including health care practices, managed care organizations, educational providers, Rapid Response Access Points, Aging and Disability Resource Centers, the NH Doorways network, 211 call center, and others.
The goals of the network include improving people’s experiences navigating the healthcare system and improving health outcomes. NH Care Connections ensures providers can quickly communicate with one another and their patients to coordinate care and ensure people have positive health outcomes. NH Care Connections reduces redundancies and overlap in workflows, ensuring providers are on the same page in real time.

Join us for this unique, in-person opportunity to meet with the NH Care Connections Team to learn about the NH Care Connections network. You will gain valuable insights about the network, steps to how your organization can participate, and why your organization is vital in addressing the needs of our New Hampshire communities.
Parking Information:
Parking is available in the Eversource (PSNH) parking lot at 780 N Commercial St. We encourage attendees to carpool or walk to the facility.
- It is also important to note that PSNH makes no warranties, guarantees or representations regarding the condition of the PSNH Property or its suitability for the planned use by the Manchester Health Department or participants of events held in the conference rooms at the Manchester Health Department.
- There will be no expectation of security of any kind provided by PSNH for the vehicles parked on site on the dates and times of use by the Manchester Health Department and conference participants. All vehicles and other articles of personal property parked or left on the PSNH property shall be at the sole risk of the owners thereof and PSNH and the Manchester Health Department shall have no liability or responsibility therefor whatsoever.
- In addition, it is understood that PSNH’s existing electric utility business and emergency storm response shall have absolute priority over the use of their parking lot.
- This agreement can be rescinded and terminated at any time prior to the event date, if needed, in PSNH’s absolute and sole discretion.
- All participants must park in the designated area below depicted on the left and NOT in the Manchester Health Department parking lot depicted on the right:
