New Mexico Coordinated Care Development Focus Groups - Las Cruces

At the request of the State of New Mexico, our organization is exploring the opportunity to stand up a statewide network of providers that would connect veterans and their families to health and human services in their home communities. During the community design and planning phase of this project, the IVMF will gather input from key stakeholders across New Mexico and develop a coordinated care implementation roadmap. Your unique perspective on the military-connected community will be essential to the creation of this network.
