We are excited to bring workshops and discussions for our youth from ages 6-19, to enhance their creativity through leadership sessions , agriculture sessions, Euntrepuernership sessions , peer conflict and resolution sessions, ettiquete sessions and more. Lunch will be served and prizes will be given!

Muhmmad Mosque 75 Inaugural Youth Conference

Activity Liability Waiver-(Includes photo release for promotional purposes)

In consideration of being permitted to participate in Muhammad Mosque 75 Youth Conference EVENT, I, for myself and my child, agree to the following:

1. I affirm neither I, nor my child, nor anyone in my immediate household, including the actual participant(s)in the EVENT, have tested positive, been diagnosed with, demonstrated any symptoms of, or have in any way been exposed to any communicable diseases, including the novel corona virus known as COVID-19 and/or any mutation or variation thereof ("COVID-19") within the past ten (10) days.

2. I acknowledge that I am aware that by entering the premises and participating in the EVENT there are risks to me and my child of being exposed to COVID-19. I am also aware such an exposure can occur directly or indirectly. If I, my child, or anyone in my immediate household has previously tested positive for

3. I understand that the sensitive topics within the certain breakout sessions may or may not stimulate strong emotion within participants and therefore each participant must be 13 and up.

4. Food preparation carries with it inherent risks that include but are not limited to health hazards resulting from improper transport, handling and storage. I understand that Muhammad Mosque 75 and the Chair of the event will be providing food and should myself or my child participate in lunch, that Muhammad Mosque 75, staff or any participating food vendors be responsible for any allergic reactions, choking, gastric or intestinal reactions on in general any harm to the participant.

5. I understand while Muhammad Mosque 75 strives to maintain everyone's safety at all EVENTS, Muhammad Mosque 75 cannot eliminate all risks. Our youth conference staff may make mistakes, when trying to eliminate or mitigate any risks. By signing this Waiver, Muhammad Mosque 75 asks you (and anyone that could legally stand in your place) to ASSUME ALL RISKS associated with you or your child’s involvement in the EVENT; 5. I hereby voluntarily agree to RELEASE, WAIVE, DISCHARGE, and COVENANT NOT TO SUE Muhmmad Mosque 75 or any vendors, staff , food providers or associations having anything to do with the EVENT, including promoters, participants, officials, and owners of the premises where the EVENT takes place (hereinafter referred to as "Releasees"). BY ACCEPTING A TICKET, YOU AGREE THAT YOU HAVE READ AND UNDERSTOOD THIS WAIVER AND RELEASE, AND AGREE TO BE BOUND BY ITS TERMS.
