Baby Music Class – April 21, 2025

I hereby assume all risks associated with participating in the Baby Music Class hosted by The Mama University, LLC and The Mom Walk Collective, including but not limited to any risks arising from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or from their potential liability without fault.

I certify that I am physically able and have sufficiently prepared for participation in this activity. I confirm that neither I nor my child(ren) have any medical conditions that would prevent safe participation. I acknowledge that it is my responsibility to ensure the safety and well-being of my child(ren) during the event.

In consideration of being allowed to participate in this activity, I, for myself, my child(ren), my heirs, executors, administrators, successors, and assigns, agree as follows:

1. Waiver and Release: I release and forever discharge The Mama University, LLC, The Mom Walk Collective, and their respective owners, officers, directors, employees, volunteers, agents, representatives, and affiliates from any and all claims, liabilities, demands, actions, or causes of action related to any injury, disability, death, property damage, or other losses sustained by me or my child(ren) in connection with participation in this activity, including those arising from the negligence of the released parties.

2. Indemnification: I agree to indemnify and hold harmless The Mama University, LLC and The Mom Walk Collective from any and all claims, demands, losses, damages, and liabilities, including legal fees, that may arise from my or my child(ren)’s participation in the event.

3. Assumption of Risk: I understand that this activity may include physical movement, interaction with other children and caregivers, and other inherent risks. I voluntarily assume all risks associated with participation, including but not limited to minor injuries such as scratches, bruises, or falls, as well as more serious injuries.

4. Medical Consent: I consent to any necessary medical treatment that may be deemed advisable in the event of an injury, accident, or illness during participation in the activity. I acknowledge that I am responsible for any medical costs incurred.

5. Photo & Media Release: I grant permission for The Mama University, LLC and The Mom Walk Collective to capture and use photographs, videos, or other media featuring me and/or my child(ren) for promotional and marketing purposes, including but not limited to social media, websites, and printed materials, without compensation.

6. Binding Agreement: I understand that this Release of Liability shall be construed broadly to provide a full and complete waiver of liability as permitted by law. I acknowledge that I have read, understood, and voluntarily signed this waiver, knowing that it affects my legal rights.
