Join us at 3924 Minnesota Ave NE at our new Community Wellness Center for our Open House on Tuesday, December 10th. To better serve MedStar Family Choice DC enrollees, the new Community Wellness Center will function as an access point to directly engage with MedStar Family Choice DC staff, receive face-to-face case management with social workers, access health and education activities, and address social determinants of health with in-person support and resources.
The MedStar Family Choice DC Community Wellness Center open house will feature representatives and demonstrations from some of the services offered including:
• Fitness classes
• Perinatal legal assistance
• Diabetes management cooking workshop
• One-on-one case managers
• Maternity and parenting support
• Wellbeing
