“A person who is defined as frail is at high risk of adverse outcomes such as falls, immobility, delirium, incontinence, side effects of Medic*tion – and admission to hospital or the need for long-term care. People with frailty can be vulnerable to dramatic and sudden changes in their health due to, for example, infection, Medic*tion change or a new environment.”

“Around 10% of people aged over 65 live with frailty. This figure rises to between 25% and a 50% for those aged over 85. It’s important that people living with frailty have access to proactive, joined-up care to maximise health and wellbeing and prevent problems arising in the first place.”

This event aims to support improvement in care and support for those living with frailty resulting in improved health and quality of life and a reduction in avoidable hospital admissions. Ensuring there is a shared understanding and knowledge of frailty across various disciplines is vital in ensuring a multidisciplinary approach with consistent best practice outcomes and patient experience.
The conference is aimed at Health and Social Care staff and others who regularly work with people living with frailty and aims to enhance existing knowledge .
