Once you have completed the questionnaire please print out and send to PO Box 674 Canterbury. Clearly mark your envelope 'Going Down To The River...To Blow My Mind.' We cannot provide a stamped addressed envelope or guarantee confidentiality. (we may also sell your information on to a third party).
