In order to start the enrollment process for your child, the following is required – items may be faxed/emailed:

3. Copy of full medical diagnostic report
4. Copy of insurance card(s), front & back
5. Copy of IEP/school diagnosis (if applicable)
6. Copy of custody documentation (if applicable)
