Life Stride, Inc is a mental health agency based in Washington, DC which provides mental health services to low to moderate-income individuals. Our agencies utilize a community model which encourages integrating services with everyday life. At Life Stride, we pride ourselves in the use of a Person-Centered lens for approaching your mental health concerns. This makes you the pilot of your treatment. * Psychiatric Medic*tion/Somatic Treatment
* Individual, Group, Family Counseling
* Community Support/Case Management
* Homeless Case Management Services including administering the VSPDAT
* Community Nurses
* Day Rehabilitation Program
* Evidenced-based Supportive Employment Program
* Operation and Management of Fourteen Community Group Homes for those with SMI
* Collateral Contacts and Forensic Support
* Assistance with applications such as; Social Security, Unemployment, & Transportation
Criteria for enrollment is simply any complications due to a mental illness or substance abuse problem. The consumer can contact DBH Access Helpline (LS & PCC can assist consumers this is a barrier) 1-888-793-4357. Accepts Medicare and Medicaid MCO and those without insurance.
