Dementia Diagnosis: What Now?
Take the guess work out of your next step. Join us for this informative class series for patients and care partners. You will learn information for daily living with your diagnosis including a practical tool to guide you through care planning and resources for maintaining independence.
Wednesdays Sept 4- 25, 2:30*-4:00pm
4 sessions addressing various topics each week:
1. Partner with your medical team & Be Informed about your diagnosis and what to expect as it progresses.
2. Legal & Financial Matters, Future Planning For Care
3. Maximize Independence & Connect Local (retaining yourself through the journey, referencing community opportunities for safety, adaptive supports, programs, etc)
4. What else should I be thinking about? – Social supports, safety, setting boundaries, grief and loss, telling your diagnosis
