Ymunwch â ni ar ddydd Mercher, 22 Ionawr ym Mhrifysgol De Cymru yn Nhrefforest, ar gyfer lansiad pecyn cymorth newydd a fydd yn cefnogi llesiant preswylwyr hŷn mewn cartrefi gofal.
Mae Age Cymru wedi datblygu’r pecyn cymorth fel casgliad o adnoddau defnyddiol er mwyn galluogi preswylwyr cartrefi gofal, eu teuluoedd a’u ffrindiau i greu lleoliad delfrydol er mwyn cefnogi eu llesiant drwy ganoli pobl hŷn o fewn eu gofal.
Ariennir y pecyn cymorth gan Lywodraeth Cymru, a chafodd ei greu yn seiliedig ar sgyrsiau gyda phreswylwyr a staff cartrefi gofal, gan ddefnyddio eu gwybodaeth a’u profiadau gwerthfawr i ddeall a bodloni eu hanghenion.
Ymunwch â ni yn nigwyddiad lansio De Cymru i:
- Dysgwch sut gallwch chi ddefnyddio’r pecyn gwybodaeth yn eich cartref gofal
- Dewch i glywed am esiamplau o ganoli pobl o fewn eu gofal, a sut gall hyn effeithio ar eu bywyd
- Dewch i rwydweithio gydag eraill a rhannu arferion gorau
Mae’r digwyddiad yn rhad ac am ddim i unrhyw un sy’n gweithio mewn cartref gofal yng Nghymru, yn cynnwys rheolwyr, cydlynwyr gweithgareddau a gweithwyr gofal; mae yna groeso hefyd i bobl sydd â diddordeb mewn cefnogi llesiant preswylwyr cartrefi gofal. Gwnewch yn siŵr eich bod chi’n archebu eich lle cyn iddi fynd yn rhy hwyr; defnyddiwch y ddolen isod.

Join us on Wednesday 22 January at University of South Wales, Treforest Campus, for the exciting launch of a new toolkit to support the wellbeing of older care home residents.
Age Cymru has developed the toolkit to bring together helpful resources to enable care homes, families and friends to create the ideal environment for supporting a resident’s wellbeing by putting older people at the centre of their care.
The toolkit, funded by Welsh Government, has been informed by conversations with care home residents and staff, drawing on a wealth of knowledge and experience to better understand and meet needs.
Join us at a launch event in South Wales to:
- Find out more about how you can use the toolkit in your care home
- Hear powerful examples of how putting people at the centre of their care can impact their lives
- Network with others and share best practice
The free event is open to anyone who works in a care home in Wales, including managers, activity coordinators and care workers; and also, those interested in supporting the wellbeing of care home residents. Places are limited, so please reserve your place using the relevant booking link below.
