Lancaster Sports Physicals

Lancaster High School and Fairfield Medical Center are offering free pre-participation physical exams.

Please book one appointment slot per athlete and include the appropriate name of the athlete booked for that time slot.

Please complete the 2025-2026 pre-participation physical evaluation form from OHSAA, which is found at www.ohsaa.org. This document must be signed and completed by the parent prior to the scheduled appointment. Athletes 15 and younger will need to be accompanied by an adult during the exam.
