About this Event

Join us for our second annual collaborative summit to discuss and identify opportunities to address the Social Determinants of Health (SDOH) (loneliness, food insecurity and tobacco/alcohol use) impacting our communities in Jacksonville.

Humana’s Bold Goal is focused on improving the clinical and social health outcomes of our members and those living in our community. We co-create solutions with community stakeholders as well as healthcare professionals to identify, screen and refer people to community resources and long-term support making health improvements easier and more attainable. Come join the Bold Goal to collectively impact the health of Jacksonville.

Military Health and Resilience was launched in 2019 to improve family resiliency, increase readiness, create higher productivity and lower healthcare costs.

By focusing on social determinants of health that impact military families most, such as food insecurity, social isolation and tobacco and alcohol use, we can provide the support and care needed to help Active Duty Service Members (ADSM), Reservists, those on National Guard and their families be ready to meet the needs of their service and country.

In 2022 MHR officially became part of the Bold Goal community, working collectively to identify and address Social Health is our priority. We look forward to working with our community partners to find solutions and collaborate to address these issues.
