To identify and discuss the different types of advance care plans that can be made, including plans which are a legal document, such as an Advance Decision to Refuse Treatment and a Power of Attorney.</li>How and where these plans can be created.</li>To explore best practice in having planning ahead conversations with others</li>To increase understanding of the responsibilities in discussing, sharing, and updating an advance care plan.</li>What plans are saved and can be accessed on Somerset’s shared integrated digital e-Record (SIDeR).</li></ul>
