The Family Resource Center is partnering with @VaccinateIndiana to offer a vaccination clinic.If you would like to participate in the clinic, please complete the online format at registration.vaccinateindiana.org/appointment/en/clinic/search
Walk-in's the day of the clinic are welcome, but we MUST have parent/guardian consent if under the age of 18. Please upload or send the immunization records for the individual who is getting vaccinated.
Please bring your insurance cards. There is no cost to those without insurance, except for the Shingles and adult RSV vaccines, as there is often a required co-pay.
