WAIVERS OF LIABILITY, HEALTH, INJURY, AND PHOTO/VIDEO
Event: I REP I HIKE AND EXPERIENCE
HEALTH WAIVER
I attest that all of the information I have provided is accurate and truthful to the best of my knowledge. I hereby affirm that I have my physician’s approval, have had a physical examination within the last 24 months, and am able to participate in physical activity. I do hereby assume all responsibility for my participation in the I REP I HIKE AND EXPERIENCE event, including the use of equipment and machinery.
I understand the potential risks of participation in I REP I HIKE AND EXPERIENCE events, including, but not limited to, heart attack, stroke, bodily injuries, and even death. While participating in events held or sponsored by I REP I HIKE AND EXPERIENCE , consistent with CDC guidelines, participants are encouraged to practice hand hygiene, “social distancing,” and wear face coverings to reduce the risks of exposure to COVID-19.
Because COVID-19 is extremely contagious and is spread mainly from person-to-person contact, I REP I HIKE AND EXPERIENCE has put in place preventative measures to reduce the spread of COVID-19. However, I REP I HIKE AND EXPERIENCE cannot guarantee that its participants, volunteers, partners, or others in attendance will not become infected with COVID-19. Participants must consent to show proof of vaccination against COVID-19 and/or proof of a negative COVID-19 test within the last 72 hours.
Participants and volunteers agree to self-monitor for signs and symptoms of COVID-19 (symptoms typically include fever, cough, and shortness of breath) and to contact I REP I HIKE AND EXPERIENCE at [email protected] if they experience symptoms of COVID-19 within 14 days after participating or volunteering with I REP I HIKE AND EXPERIENCE .
INJURY WAIVER
I understand and acknowledge that participation in I REP I HIKE AND EXPERIENCE events involves physical activity and carries inherent risks, including but not limited to falls, contact with other participants, equipment-related injuries, and the effects of weather conditions. I fully assume all risks, known and unknown, associated with my participation in any I REP I HIKE AND EXPERIENCE event, whether caused by my own actions, the actions of others, or the conditions of the event venue.
I hereby release, discharge, and agree to indemnify and hold harmless I REP I HIKE AND EXPERIENCE , its members, owners, employees, contractors, sponsors, attorneys, agents, successors, and assigns from any and all claims, liabilities, demands, actions, causes of action, costs, and expenses (including legal fees) arising out of or related to any injury, illness, disability, or death that may occur as a result of my participation in the event.
I acknowledge that I REP I HIKE AND EXPERIENCE is not responsible for any medical expenses, injury-related costs, or loss of income that may arise from my participation. I understand that it is my responsibility to consult with a physician before engaging in physical activity and to determine whether I am in proper health to participate.
By signing below, I acknowledge that I have carefully read this Injury Waiver and fully understand its contents.

I hereby give my consent to be photographed and/or videotaped by

I REP I HIKE AND EXPERIENCE and its authorized representatives. I also consent to the use of my identity and likeness in I REP I HIKE AND EXPERIENCE ,irepb and our partners' social media and promotional materials, including the use of my likeness for I REP I HIKE AND EXPERIENCE website.
It is understood that this agreement is binding on myself, my heirs, relatives, executors, administrators, agents, attorneys, insurers, and assigns should I suffer injuries or death.
