Hypertension Self- Management Education and Support Program
This hypertension education workshop (in person or via zoom) provides an overview of hypertension, healthy food choices, reducing risks, avoiding complications, healthier choices to improve blood pressure management. Participants who attend the workshop can schedule a follow-up one-on-one appointment with a health counselor to discuss individual hypertension management concerns.
A healthy lunch will be provided for in-person participants.
