Hello Monmouth County Partners!

Thank you for participating in the Community Forum in June - a critical aspect of the Community Health Assessment (CHA) process - and for expressing your interest in joining the Community Health Improvement Plan (CHIP) Steering Committee. Your feedback throughout this process has been invaluable and has laid the backbone for the collaborative CHIP. We would like to share with you how we will use the data we have gathered, and the ideas you have shared, to make a plan to improve the quality of life for everyone in Monmouth County. We would also like to discuss the ways in which the HICMC's CHIP and partnership can be beneficial to your work towards a healthier, more whole Monmouth County.
