You're invited to a free NHS event, funded by Healthwatch Hillingdon and the London Borough of Hillingdon. Join us for a day filled with insightful health information, interactive workshops, and opportunities to engage with our incredible healthcare teams.

You'll leave the day with information and opportunities to be healthier than ever before, find resources to support your well-being, tips and tricks for a balanced diet and ways to get active this summer.

What to expect:

• Find Well-being and Healthy Living Support: Stay well by discovering various well-being and healthy living resources available in your local community.
• Get Active You can view the facilities and explore different ways to keep fit in the gym and across the borough.
• Learn about Diabetes: Support in managing a healthy lifestyle while understanding and navigating the challenges of diabetes to ensure your well-being.

• Access Health Checks: Receive essential health checks, including blood pressure, weight/height measurements and more, ensuring you stay on top of your well-being.
• Engage with Our Amazing Primary Care Network Teams and Charity Partners: Meet our dedicated professionals, including Clinical Pharmacists, Social Prescribing Link Workers, Health and Well-being coaches, and others, who are here to support your health journey.

Welcoming patients from Synergy Primary Care Network (PCN). Synergy PCN is a family of practices that includes:

• Belmont Medical Centre
• Brunel Medical Centre
• Central Uxbridge Surgery
• Hillingdon Health Centre

A PCN is like a close-knit team of healthcare providers. They work together to ensure you receive the best possible care and support, making your health journey smoother and more convenient.
