* Photo ID, Social Security Cards or immigration paperwork for all persons wanting coverage
* Proof of address
* Proof of income, such as a month of pay stubs, recent W-2, tax returns or Social Security statements
* If enrolled in Marketplace during the last year, bring account information, including user name, password and Marketplace ID
* Policy number for any current health insurance plans covering you and/or members of your household
