HEAL-D Programme: Type 2 Diabetes Self-Management

📍 African Caribbean Centre
🗓 Fortnightly sessions | 8 weeks

HEAL-D is an evidence-based, culturally tailored programme designed to support Black African and Caribbean adults living with type 2 diabetes to better manage their condition and improve their wellbeing.

About the Programme

HEAL-D was developed in partnership with people living with type 2 diabetes of African and Caribbean heritage, alongside faith and community leaders and healthcare professionals. This collaborative approach – often referred to as co-production – means the programme is shaped by lived experience as well as clinical expertise.

Through ongoing community engagement, we continue to listen to participants and adapt the programme to ensure it remains relevant, practical, and culturally meaningful.

What to Expect
  • A supportive, community-based learning environment
  • Practical tools to help manage type 2 diabetes day-to-day
  • Culturally relevant discussions around food, lifestyle, and wellbeing

HEAL-D promotes healthy behaviour change and increased confidence in diabetes self-management, helping participants take positive steps towards better health.
