HEAL-D is an evidence-based, culturally tailored programme designed to support Black African and Caribbean adults living with type 2 diabetes to better manage their condition and improve their wellbeing.
About the Programme
HEAL-D was developed in partnership with people living with type 2 diabetes of African and Caribbean heritage, alongside faith and community leaders and healthcare professionals. This collaborative approach – often referred to as co-production – means the programme is shaped by lived experience as well as clinical expertise.
Through ongoing community engagement, we continue to listen to participants and adapt the programme to ensure it remains relevant, practical, and culturally meaningful.

- Culturally relevant discussions around food, lifestyle, and wellbeing
- Evidence-based guidance informed by research, clinical knowledge, and patient experience
HEAL-D promotes healthy behaviour change and increased confidence in diabetes self-management, helping participants take positive steps towards better health.
Commitment
This programme runs over 8 weeks, with fortnightly sessions.
👉 Participants are asked to commit to attending all sessions to gain the full benefit of the programme.
