Come tour our center, meet our team of care providers, and learn how Habitat Health supports adults 55+ who want to stay independent and connected—while also supporting caregivers with extra peace of mind.
This is a relaxed, drop-in event. Bring a friend, a family member, or a caregiver.
What to Expect
- Guided tours of the PACE Center
- Meet the care team (clinical staff, social work, care coordination)
- Wellness and mobility demos (PT/OT-style tips, nutrition and education)
- Light refreshments
- Activities and community connection (games, photo moment, and more)
- Questions + next steps with our Outreach & Enrollment team
