The SCDHEC Best Chance Network Gap to G.O.A.L.S Learning Collaborative participating practices are working:
1) To accelerate the spread of evidence-based best practices and support practice engagement with quality improvement methods.
2) To increase breast cancer screening referrals and cervical cancer screenings being performed by providers in your area.
3) To support practices in implementing direct claims entry.
4) To create a community to share best practices and shared learnings to improve systems for optimal BCN participation.
In our efforts to develop a sustainable, comprehensive framework for reducing breast and cervical cancer health disparities, we have developed a series of in-person meetings, office hours, and an action planning period. You and the team will have the opportunity to discuss and review data, innovative strategies, and lessons learned to overcome any identified problem areas and communicate with field experts.
