Join Future Matters Islington for a free in person event about how you can get support planning your future care wishes, through power of attorney for financial and legal affairs and health and welfare and other advance care planning

Advance Care Planning

Advance care planning is a process of discussing and preparing for future decisions primarily around your medical care if you become seriously ill or become unable to communicate your wishes. It also includes ensuring your preferences are noted and helps you to think about what is important to you and to make your wishes, feelings, beliefs and values known to both your loved one but also to those looking after you including Healthcare Professionals.




Setting up a ‘Lasting Power of Attorney’

Lasting Powers of Attorney (LPAs), are legal documents that allow you to choose one or more people to help make decisions or to make decisions on your behalf, should you be unable to do so. LPAs are a simple way of protecting your health, your finances, and the people you trust.

LPAs can be expensive to set up, but the Future Matters Islington service, run by Gentle Dusk in partnership with Age UK Islington, can provide free support to help you put one in place.



How can I get further support following the presentation?

Future Matters Islington offers free one-to-one support for putting in place an LPA and other advance care plans. These will enable you to appoint one or more people to help you make decisions or make decisions on your behalf to do with your health and financial affairs.

Who is it for?

This event is open to anyone with an interest in learning more about planning for their future.

The Future Matters Islington service is available to all Islington residents or those with an Islington GP with an interest in learning more about planning for their future; there are no health or financial criteria. To make an LPA you must be 18 or over and have mental capacity.
