Bolivar Family Care Center will offer free scheduled sports physicals for middle and high school athletes Tuesday, Aug. 6 & Wednesday, Aug. 7, 8 a.m.-4 p.m. Please call Bolivar Family Care Center at 417-326-6021 to schedule your appointment. The clinic is located at 1240 N. Butterfield Road in Bolivar.The sports physicals are free on the specified dates and times listed only. On all other days, walk-in or scheduled appointments for sports physicals are $44 (payment is due at the time of service).
For student-athletes who also need a wellness examination, the sports physical paperwork can be completed during the wellness appointment. Most insurance plans cover a wellness exam once per calendar year at no cost. Call the clinic to schedule an appointment and for more information.
