We are pleased to inform you that the Pope County Health Department will be providing a flu vaccine clinic at Oakland Heights on October 17th at 12:30. Please note, only the injectable (inactivated) flu vaccine will be administered. No live nasal spray vaccine will be given. To participate, please complete and return the consent form (front and back) and FERPA form that were sent home with your child. Forms are due on October 15th. It is important that we receive the completed forms by the due date to ensure your child can be scheduled for vaccination.
