Fitness at Ignite

Ladies, join us for an 8 week fitness class on Tuesdays from 6:30pm-7:30pm. Bring your mat and small hand weights if you want to use them and be ready for a dynamic workout at your own pace.

Release of Liability Statement

By participating in the above-named event, I acknowledge and agree to the following:

1. Assumption of Risk: I understand that participation in this event may involve inherent risks, including but not limited to physical injury, accidents, or other unforeseen events. I voluntarily assume all risks associated with my participation.

Release of Liability: I hereby release, waive, and discharge Gwen Tyson/Intentional Life Consulting, PLLC and Ignite Church and its representatives, employees, volunteers, and agents from any and all claims, liabilities, demands, or causes of action that may arise from my participation in this event, whether caused by negligence or otherwise.

3. Medical Treatment: I consent to receive medical treatment that may be deemed advisable in the event of injury, accident, or illness during this event, and I assume all responsibility for any medical expenses incurred.

4. Photography and Media Release: I grant permission for Gwen Tyson/Intentional Life Consulting, PLLC and Ignite Church to use photographs, video recordings, or other media taken during the event for promotional purposes without compensation.
