Recognising and Responding to Frailty: Using the Community Frailty Hub to Support the Right Care, Right Time
This free CPD session, held at the Post-Graduate Education Facility, Scarborough District Hospital, invites all staff to explore the concept of frailty and how to recognise and manage it effectively in the community.
Delivered by Dr Elizabeth Swain, Speciality Doctor in Frailty and based at the North Yorkshire Community Frailty Hub, the session will use real-world case studies to examine what frailty looks like in practice, when and how to make appropriate referrals, and how the Hub supports safe, coordinated, and person-centred care.
The event aims to enhance clinical understanding, build confidence in decision-making, and strengthen links across community and acute services to ensure patients receive the right care, at the right time, in the right place.
