The Peer Models for Health Learning Collaborative is composed of clinical and community-based peer workers and peer supervisors striving to reduce health disparities in North Philadelphia's Health Enterprise Zone (HEZ) and across every corner of Philadelphia.

Integrated Peer Models for Health (IPMH)
The Health Federation of Philadelphia's new Integrated Peer Models for Health (IPMH)Program supports the growing community of Community Health Workers, Certified Peer Specialists, etc., and their supervisors working within the North Philadelphia Health Enterprise Zone (19120 -19123, 19125, 19130 - 19134 19138 19140, 19141,19144) to address a myriad of social determinants of health that contribute to the high prevalence of uncontrolled chronic physical and mental health conditions within our communities.
Through our Peer Models for Health Learning Collaborative, IPMH strives to provide training, discussion, and networking opportunities that support the critical work of programs employing peers, in order to build systems that effectively and skillfully address the needs of Philadelphia's vulnerable communities.
