About this Event
(CW OHT)'s 3rd annual Navigation Conference is designed to inspire and support the community of navigators.
With speakers from Health811, WellFort Community Health Services and more, this conference seeks to facilitate access to care and smooth transitions between settings, and foster collaboration and learning among healthcare providers, navigators, and organizations.

This event can help you unlock your potential, foster meaningful connections and drive positive change in your community.
Learn more about the CW OHT's commitment to the future for navigation activities that will make a meaningful difference in working towards navigation for all, equitable access to care, system integration, and practical navigation tools and workflows that support primary care attachment.
We want to bring together system navigators, health and community partners, as well as strengthen navigation supports and promote shared learning across the region.
This event's sessions will include:

- Exhibitor booths featuring over 20 agencies from across the Central West OHT

Private organizations / for-profit organizations can purchase tickets for $25, here on Eventbrite (not at the door).
