Create Your Own Advanced Aesthetic (Private One-on-One) Training

We are proud to introduce a new concept to aesthetic training. Now, you can choose your own learning experience with the same Private One-on-One training format. You will be able to choose 1 of the following training topics below depending on your previous training and scope of practice. We will review the specific procedure, application, anatomy, procedure variations, safe procedure techniques, hands-on practice, and adverse event management.

***This training requires you to bring your own model(s) to this training so you may follow their progress. We do not provide models for this training.***

-Chemical Peels

-PRP (skin and hair restoration)

-Neurotoxins

-Fillers

-Cannula (Micro-Cannual) for filler

-The Business of Aesthetics

Certificate awarded with class completion.

Prerequisites:

1. Medical professional with an unrestricted, active license in the state of Massachusetts.

2. Must have completed an Introduction to Neurotoxin and Introduction to Dermal Fillers training. If taken outside The Roosevelt Center for Advanced Aesthetics, please submit certificates of completion upon registration.

3. You will be able to choose 1 of the following training topics below depending on your previous training and scope of practice. This

4. For Cannula (Micro-Cannula) training: 6mo-1yr of continuous injecting experience with dermal fillers as this is an advanced class.

Photography and Videography Policies: By Signing up for this training you unequivocally agree to the following.

I hereby irrevocably consent to and authorize the use and reproduction by Roosevelt Lewis and The Roosevelt Center for Aesthetics and Cosmetics, LLC and his affiliates, or anyone authorized by any of them, of any and all photographs, electronic images or video footage of me taken by Roosevelt Lewis and The Roosevelt Center for Aesthetics and Cosmetics, LLC, or that Roosevelt Lewis and The Roosevelt Center for Aesthetics and Cosmetics, LLC, has in its possession, provided either by me or by a third party (collectively, Images) for the purpose of informing the medical profession and the general public about medical aesthetics and wellness procedures and techniques without compensation to me.

Such use shall include, but not be limited to, distributing the Images via print, visual and electronic media, specifically including the Roosevelt Lewis and The Roosevelt Center for Aesthetics and Cosmetics, LLC website and social media sites such as but not limited to YouTube, Facebook, Instagram, Tik Tok and Twitter. The Images (including any photographic negatives) shall be the sole property of Roosevelt Lewis and The Roosevelt Center for Aesthetics and Cosmetics, LLC. Roosevelt Lewis and The Roosevelt Center for Aesthetics and Cosmetics, LLC also shall have the right to use my name in connection therewith if it so chooses.

I hereby waive any right to inspect or approve the finished product, photograph, video, DVD, CD-ROM or matter that may be used in conjunction therewith or to the eventual use that it might be applied.

I hereby release, discharge and agree to hold harmless Roosevelt Lewis and The Roosevelt Center for Aesthetics and Cosmetics, LLC and its affiliates and their respective representatives, assigns, and employees, and any person acting under their permission or authority, from and against any claims whatsoever in connection with the use of my Images and name and the reproduction thereof as stated above, including any claim for payment in connection with distribution or publication of the video and/or photographs.

I hereby warrant that I am eighteen years of age or older, and competent to contract in my own name insofar as the above is concerned.

I have read and understand the foregoing release, authorization and agreement, and enter into it knowingly and voluntarily.
