New Life International Seventh-day Adventist Church is proud to host in partnership with the Florida Department of Health and Health Hero Florida a FREE COVID-19 Vaccination Clinic on Saturday, June 26, 2021.
The Pfizer COVID-19 vaccine will be available to all individuals age 12 or over. Individuals 12-18 years of age must be accompanied by a guardian.
First Dose: Saturday, June 26, 2021
TIME: 9:00 AM TO 1:00 PM
LOCATION: 6429 Atlantic Blvd. Jacksonville, FL 32211
Appointment Details
- The Pfizer COVID-19 vaccine will be available to all individuals 12 years of age or older. Individuals 12-17 years of age must be accompanied by a guardian to obtain the vaccine.
- Please check in when you arrive. Bring your ID along with your medical insurance.
- Download and complete the Florida Department of Health Screening and Consent form before coming to your first appointment. Remember to bring it along with you for a faster check-in process. If you are unable to print this form or forget to bring it with you, forms will be available for you to complete.

- Don't forget a face covering. Wearing it throughout your visit is requested.
- Wear a short sleeve shirt to make getting the shot easier and faster.
- Walk-ups accepted

If you are experiencing COVID-19 symptoms:
- Don't go to the clinic site.
- Contact your primary care provider for next steps.
Tips for you second dose:

- Don't forget to bring your Florida Shot Record along with you when the time comes to receive your second dose.
