Join the CommunityCares team on Tuesday, April 15th for our On the Road Series with an in-person onboarding workshop for any non-healthcare community-based organizations (CBOs).
Whether your CBO is already connected to the CommunityCares program, or you've never heard of it, this free workshop provides an opportunity to get individualized, on-site support for your organization to best utilize Arizona’s statewide Social Determinants of Health (SDOH) closed-loop referral system.
