Welcome to our Community Health Fair Vendor Event! Join us at 200 W Southern Ave for a day filled with health and wellness.

All vendors should be present at 9 am to set up

• Placement fee of $50 is to be received 7 days before the event
• Please mail your check to 200 W Southern Ave Phoenix AZ 85041
• Limited amount of tables First Come First Served Basis Please provide the number in your group
• Chairs Provided
• If you require a private room for your services please indicate upon signing up
• The following items are prohibited in the premises and not allowed for distribution or advertising: Alcohol, jewelry/charms,cosmetics.
• This is a Health Services related only vendor registration. No other vendors allowed.
