Pinnacle Fertility invites you to an evening of connection, celebration, and community at our newly remodeled Glendale clinic. Join us on Friday, September 12, from 5:00 – 7:00 PM for Cocktails & Bites at Pinnacle Fertility – Glendale Clinic (17612 N 59th Ave, Suite 100, Glendale, AZ 85308).

Terms & Conditions: To be eligible for the raffle, attendees must preregister for the event and attend the Pinnacle Fertility Arizona Cocktails & Bites on Friday, September 12th, at any time between 5:00 and 7:00 pm PST. Four separate fertility service credits will be awarded through a random drawing, including one grand prize credit of $10,000, one credit of $5,000, and two credits of $2,500. Winners will be selected after the open house and notified by email and phone call on Monday, September 15. Attendance at the time of the drawing is not required to win.

The fertility service credits must be used within 12 months of the event date and will be voided if not redeemed and fertility treatment has not begun by September 12, 2026. Expired credits will not be honored under any circumstances. Each credit has no cash value, is non-transferable, and any services exceeding the credit amount will be the sole responsibility of the recipient. Eligibility for services is subject to clinical consultation and approval.

Patients are required to provide accurate personal information and sign all required informed consent forms before treatment begins. Pinnacle Fertility Arizona and its affiliates provide fertility services in accordance with applicable laws and clinical guidelines. While every effort is made to achieve the best outcomes, results may vary depending on individual circumstances. Payment is due at the time of service unless otherwise arranged, and while insurance claims are supported, they remain the patient’s responsibility. Cancellations require at least 24 hours’ notice to avoid no-show fees. All personal information is processed, stored, and used in compliance with HIPAA and other applicable privacy laws.

Employees of Pinnacle Fertility, Inc. and their immediate family members are not eligible to participate. Limit one entry per household. Additional terms may apply and are subject to change without notice.
