Welcome to CardioRenal Vision's 1st Annual Care Transitions Symposium! Join us for a day of learning and networking at the beautiful InterContinental Bellevue at the Avenue, an IHG Hotel. Our symposium will feature a panel of local industry experts to discuss the latest trends, advancements, and barriers in care transitions.

From acute care to home health and hospice, healthcare is constantly changing. As new federal and state policies are voted into law that aim to improve healthcare quality and safety, healthcare organizations must be innovative and flexible. These policies, including new staffing policies for skilled nursing facilities, will impact both acute and post-acute medical facilities, making it more difficult for them to meet and exceed benchmarked quality outcomes, requiring them to carefully choose up and downstream partners who have the same dedication to quality and safety achievements.

We will spend the day discussing innovative and transformative impacts in both acute and post-acute care. We will hear about how EvergreenHealth and Overlake Medical Center and Clinics entered into a joint venture to create the Eastside Health Network (EHN). This network develops and implements evidence-based, data-driven clinical initiatives to improve outcomes, increase efficiency and reduce costs in the inpatient and outpatient settings. We will hear about current and future federal and state policies impacting skilled nursing facilities and ultimately impacting the ability of acute care hospitals to quickly and safely discharge their patients to the nursing facilities. We will hear about the use of an innovative post-acute provider EMR that incorporates AI to improve provider efficiency and patient outcomes. Finally, Dr. Ahmed will close us out as he discusses how CRT has been able to improve the experience of CHF and CKD patients through close collaboration with hospital and community cardiologists and nephrologists.We will be able to take time to focus on where we are, where we are going, and what we need to be doing about it.
