We will hear about current and future federal and state policies impacting skilled nursing facilities and ultimately impacting the ability of acute care hospitals to quickly and safely discharge their patients to the nursing facilities. We will hear about the use of an innovative post-acute provider EMR that incorporates AI to improve provider efficiency and patient outcomes. Finally, Dr. Ahmed will close us out as he discusses how CRT has been able to improve the experience of CHF and CKD patients through close collaboration with hospital and community cardiologists and nephrologists.We will be able to take time to focus on where we are, where we are going, and what we need to be doing about it.
