A doctor's order is not needed for women age 40 and older. For women younger than 40, a doctor's order is required.
Appointments are preferred but walk-ins will be accepted. To schedule an appointment, call Premier Health at (855) 887-7364 & select Option 1. Most insurances are accepted. No insurance? No problem. Call (866) 838-8973 to apply for Premier Health's Voucher program.
