Info: This session aligns with the NHS's 10 year strategic vision to shift care out of hospitals and into communities - referred to as the "left shift". The goal is to deliver collaborative, proactive, preventative, and integrated care closer to where patients live, ensuring sustainability, equity, and efficiency across the system. Each speaker will offer insights into practical implementation, highlighting innovation, local leadership and integrated care pathways. We will hear from experienced leaders about innovative models being delivered on the ground, followed by an open Q&A session with attendees.
